The feel of the toothbrush, the taste of toothpaste, or the bathroom lighting may play a role. Every child is different; difficulty with toothbrushing, on its own, does not indicate a diagnosis or unwillingness.

What might make toothbrushing difficult?

To better understand your child, consider which part feels difficult. For example:

The toothbrush bristles may feel uncomfortable when they touch the gums or the inside of the mouth.

The taste, smell, or foam of the toothpaste may be unpleasant.

Water, the mirror, bright lighting, or bathroom sounds may be distracting.

Not knowing when the activity will start and end may cause anxiety.

These are only possible examples; the same reasons do not apply to every child. Several factors may play a role at once, and the difficulty may change over time.

Respect your child's comfort

First, try to understand at what point your child finds it difficult. If your child communicates by speaking, pointing, showing pictures, or in another way, give them time to communicate. Some families try explaining beforehand what will happen or introducing the toothbrush to the child. These are examples; there is no single approach that works for everyone.

If your child is visibly distressed, take a break. Rather than forcing toothbrushing to continue, shaming your child, or punishing them, respect their safety and boundaries.

When should you consult a dentist?

If you suspect mouth pain, if brushing difficulties continue, or if a dental visit causes anxiety, talk with a dentist. You can share what bothers your child and how they communicate before the appointment.

A dentist should assess individual issues such as brushing technique and frequency, fluoride, or choosing oral-care products. This article is not a substitute for an examination or individual oral-care advice.

Sources

NHS — Supporting an autistic child

Autism Speaks — Autism and Dental Care Guide

PAAutism — Dental Health Resources

Author: Ufuk Çelik, researcher · Last updated: October 5, 2026.
